    Program Overview

    The Injector Collective (located at 139 Hunter's Grv Ln Ste 125, Lehi, UT 84043) offers an SMS messaging program to keep you informed about your care. By opting in, you consent to receive recurring text messages including:

    • Appointment confirmations and reminders
    • Post-visit follow-up communications
    • Promotional messages about services, events, and special offers
    • Membership and billing notifications
    • General practice updates

    How You Opt In

    We collect your SMS consent before any messages are sent through the following methods. At each opt-in point, the consent checkbox is unchecked by default — you must actively check it to opt in.

    1. Online Booking Form

    When booking an appointment at injectorcollective.com/book, you complete a multi-step booking form. On the "Your Info" step, the following consent checkbox and disclosure is presented:

    "I agree to receive recurring SMS messages from The Injector Collective (including appointment reminders, follow-up care instructions, membership billing notifications, and promotional messages).     2. In-Person Digital Check-In

    At our physical location (139 Hunter's Grv Ln Ste 125, Lehi, UT 84043), clients check in via a self-service kiosk at injectorcollective.com/check-in. During the review step, the same SMS consent checkbox and identical disclosure language is displayed. Clients must actively check the box to opt in.

    Consent Language

    At every opt-in point (online booking form and in-person check-in), the following exact disclosure is presented to users:

    Message frequency varies based on your appointment schedule and interactions with our services. You may receive messages for each appointment you schedule.

    Opt-Out Instructions

    You may opt out of receiving text messages at any time by replying STOP to any message from us. After opting out, you will receive a one-time confirmation message and will no longer receive texts from The Injector Collective.

    Re-Subscribe: If you have opted out and wish to receive messages again, you may re-subscribe by texting START to our number or by opting in again through our booking form or check-in process.

    Supported Carriers

    SMS messages are supported on all major US carriers including AT&T, Verizon, T-Mobile, Sprint, and others. Carriers are not liable for delayed or undelivered messages. Delivery is subject to effective transmission by your mobile carrier.

    Third-Party Sharing

    Opt-in data is never shared with third parties.

    All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
